Tathata Golf Junior Group Program Registration Form

This program runs from April 1st through September 30th. Junior golfers may begin this program anytime between April 1st and August 31st. The program ends on September 30th. Please complete this form and click the SUBMIT button (bottom) to send to us. We will contact you and let you know when your account is set up and payment instructions for the season membership fee. Once this is done, you may schedule your junior’s first group session.We will contact you if we have any questions. Please complete a separate form for each child you are registering. Thank you.
